(that is, randomizing groups to be vaccinated and not vaccinated). That was done with the phase 3 vaccine studies (albeit all patients had to be previously uninfected.)

Now that those studies are done, it would indeed be unethical to do another prospective study with unvaccinated individuals, but they can do a large-scale, long observation study looking back retrospectively where they can adjust for differences between the groups
