y sit down and have rational discussions over specific things that could possibly be changed that might prevent this sort of thing.

But it seems that everyone talks in generalities.

In this case, it seems likely to me that the officer was either trained poorly or had some sort of mental issues.

As far as possible remedies, perhaps working on changing the culture such that fellow officers would feel duty bound to try and rein in brothers who are stepping over the line could prevent future occurrences like this one.
