decent into a cratered state, largely due to how unprepared and overwhelmed he was initially and then perpetuated by numerous tactical decisions/approaches and fundamental limits in important skill sets required for success at the job.

Typically, the head coach is deemed responsible for both the program and its success. In this case obviously more so than most.
