"Redshirt" is sitting out without regard to suffering any injury or other medical hardship. You are correct that if you engage in ANY competition after the start of the academic year, you cannot "redshirt".

"Medical Hardship" refers to injury situations. Those allow you, for segmented sports, to play up to 30% of the team's scheduled competition dates, but only in the first half of the season.

Then, there is the "Medical Exemption" which is a career ending injury, but allows the school to continue to honor your scholarship, but not play, so you can finish your education on the school's dime, without counting against scholarship limits.
