It depends on what an insurer and insured are willing to agree to.

Permitted? Well, yes.

No, but taxation should be ended.

6. Should premiums be subsidized?
By...the government? No.
7. Should you be able to see you present doctor?
If you can agree to an appointment with your doctor, then of course.
