No.

It depends on what an insurer and insured are willing to agree to.

3. Should drug formularies be permitted to define what medicine you take? What if it is contrary to what your doctor has prescribed?

Permitted? Well, yes.

4. Should medical premiums be tax deductible (either as pre tax dollars or deductions)?

No, but taxation should be ended.

5. Should healthcare costs be tax deductible (either as pre tax dollars or deductions)?

No, but taxation should be ended.

6. Should premiums be subsidized?

By...the government? No.

7. Should you be able to see you present doctor?

If you can agree to an appointment with your doctor, then of course.
