Human error is always going to happen that is the nature of the beast. So yes this kind of Mistake Can and Does happen and Will continue to happen in all things that Involve Man or Machines nothing in infallible. There are no absolutes in life. A horrific mistake that can't be undone. But at least they are not trying to cover it up and are being forthwith about the situation.

Please explain why this message is being reported.
