Washington - not much to compare to given only two Presidents, but his putting down the Whiskey Rebellion resulted in a few American deaths.
Lincoln - the easiest one to explain on the list. His war for power resulted in the deaths of over 700,000 Americans.
GW - his illegal wars (on deceptive bases) led to the deaths of over 7,000 Americans
They are responsible for those deaths. It has nothing to do with them "just being president" when those deaths occurred.
