Why get preventative care when any bad result will be fully paid regardless. A big reason why employer paid insurance works well is the incentives they have built. Giving people money to go through various programs designed to reduce healthcare costs. The first $3k of healthcare being out of pocket to get people to try to reduce their medical costs. Etc.
