Insurance companies are going to get their money. If Joe doesn't have insurance and goes to the ER, then Jane and Mary and Fred pay Joe's bill in the form of higher premiums.
So, what do you do? You require people to have insurance so Jane and Mary and Fred don't have to pay for Joe.
Ah, but the real solution is to make sure Joe, Jane, Mary and Fred don't smoke or drink too much. You make sure they eat right and not too much.
Then, a lot of the costly chronic disease goes away. Less really expensive tertiary care is needed. In short, costs to the system go down.
This is what other countries do better than us.
Insurance companies have no incentive to reduce the overall cost. On the contrary, the more care they can cover, the better their bottom line.
Fat people, hell yes!
That is why the government needs to be involved. As horrible and inefficient as it is.
