is medical coding. There has been a game going on for some time to maximize the facilities margins by creative/aggressive coding.
The issue is there needs to be some transparency before a visit (obviously if it is an emergency you may not have time to weigh your care options).
Time and time again you get an EOB from the insurance company that is not what is expected by your benefits because of the art of medical coding.
