Job two - yes, fee for service if paid in cash. I no issue with that idea.
Job three - Join the rest of the planet and set up single-payer system that funnels through tax system. Increase your tax rate some 3% or perhaps whip a little ntl sales tax on products and services and you have your health care covered.

GEE, everyone in the world does it. But US Capitalists claim to have a better system. They don't. They have a failed system.
