Look into history people the welfare system commodities and health care has been in existence for a long time what LBJ did was to include a lot of people that were left out predominately white people and people of color which were in the similar situation as the Appalachian people. Because of his Civil rights bill the laws got to the point that it destroyed families because the anti civil rights people were not going to help a Black family (or any family unless the man or father was not present. there was a certain criteria that had to be met for assistance which resulted in single parent families and decline in the American family.
