there's no way its made its way through the health insurance processing channels to even determine coverage & payment or denial yet.

****, I go to the doctor for a routine annual physical and its like 3 months before I get a bill and everything processes through insurance

Unless he just flat-out didn't have health insurance at all. Thats a possibility...he's probably not technically employed anymore, and even when he was fighting he was probably an independent contractor with no benefits. The UFC may have provided some sort of accident policy if he were hurt in the ring, but maybe he didn't carry personal health insurance otherwise.
