During the Star Spangled Banner, the President, like all civilians, should stand at attention with their right hand over their heart, according to the U.S. Code. If in military uniform, the President would render a military salute. The President, as Commander-in-Chief, is always saluted, but during the national anthem, the hand-over-heart gesture is the appropriate civilian protocol.
