It's not surprising that when you really need one to work it doesn't. Either coaches don't know how to coach it or players don't do what coaches teach. Either way it's a coaching issue.

Look no further than the majority of pick n rolls. The "pick" almost never happens. The roll doesn't happen effectively either. It's comical to watch. And a sad indictment of fundamentals today.
