reason that is important is because if he says, or the first nurse says, it's his health, how will they then make the case that he may be too weak to run a campaign, but not too weak to run the country?

If he says he wants to spend more time with his family, no one will believe him.

Is there a plausible reason he could say he doesn't want to run but wants to remain president for six more months?
