You have to pay the cast, set, production, etc.. just like any other reality TV show, gameshow, or sitcom/drama/comedy.

As long as they get the advertisers (enough to be profitable) it will stay around.

Live sporting events are DVR proof when it comes to commercials, however, you can't really syndicate re-runs which is a disadvantage.
