Because of no proper controls, so then the authors (or compilers of papers) use their biases to build towards a conclusion.
The sad part is this becomes political versus people really trying to understand what is causing the variations in outbreaks prior to vaccinations.
Some are fairly obvious (the early outbreak centers). After that is not so obvious, after even adjusting some for population density.
