What he is insinuating is that you have "followers" that aim to please you and you assign value to yourself because of these "followers", and in his view, pleasure comes from genitalia. Therefore, in his view, your pleasure comes from your followers enjoying your ballsack because he can't understand pleasure coming from something else.
