At my HS job had a new hire they were training to replace me. We were putting cardboard into the compactor/dumpster and he tilted one of the 4-wheel carts tp empty its contents into the dumpster and somehow followed it in. I was inside the dock preparing to compact the load when I heard him yelling.

He didn't survive the probation period.
