The States can decide the laws and penalties for breaking them as the citizens deem appropriate.

Historically, that has been penalties for those performing the abortions... such as suspensions or revocations of medical licenses &/or civil penalties &/or criminal penalties.

As with other crimes penalties escalate for repeat offenders.
