there would have to be sufficient basis the juvenile office to open an investigation and for a judge to determine both that the kid staying in the home is contrary to his welfare and that the juvenile office made reasonable efforts to prevent the need for removal (or more likely here, an emergency impacting his physical health).
it's not an indication of guilt, but it is multiple layers of people determining something very not right happened.
