The conventional economic wisdom has been a single payer would be the lowest cost system.

However, in 2018 a single payer system for California was proposed by Dems in the California legislature. The bill was abandoned after cost studies showed the costs were prohibitive.

Bernie Sanders' medicare for all proposal has been costed out by at least three different think tanks in Washington, D.C. The projected costs for the first decade fall in the 30 to 36 trillion range.

It appears there's a substantial disconnect between economic theory that suggests single payer is the least cost system and the results of the cost studies.
