medical reasons.

I had a HS Classmate who got an Appointment to West Point at the same time I did. His father, grandfather and great grandfather were West Point grads. He was an All State in cross country and track. He failed his physical because it showed he had asthma and his Appointment was pulled. ... He never served in the military. Did he 'refuse to serve'?

I had a boss in the Army (A LTC who had just graduated from the Command and General Staff College.) He was diagnosed with diabetes and removed from the military. Did he 'refuse to serve'?
