It can come down to people that think being gay is a sin and feel the need to physically harm someone for being gay (e.g. gay hate crimes). It may come down to a feeling that if another gay person is around your kid, that may make your kid gay. It may be like saying that because gays get married that it somehow affects your marriage. It could be parents that kick their kids out of the house when they come out of the closet. It could be a person that is emotionally trashing a person for being gay...gay jokes, name calling etc. And finally, yes, there are some that do believe if you do not follow the full "gay agenda" you are a homophobe.
I have found it a bit interesting that I knew a few people that came off as homophobic to only learn they were gay years later.
