It is bargaining power. Insurance companies, prescription benefit managers and institutional providers have unexposed fees that the consumer never sees. Admin fees, rebates, preferred pricing, etc.

It is so complex that Trump was caught off guard two weeks ago but apparently he has it all figured out now.
