There isn't anything in that McKinsey summary which contradicts the HMO Act as the primary driver of higher healthcare costs, either. In fact it affirms characteristics such as price insensitivity which are natural expectations when patients aren't paying directly.

Before the HMO Act medical insurance only covered emergencies, surgery and hospitalization. Kennedy was the primary sponsor of the Act but when the natural consequences appeared he began to disown his own handiwork and criticize HMOs.
