both by Medicare/Medicaid and by private insurers. They are denied coverage to begin with, they are dropped from coverage on some rare cases (at least before Obamacare passed) and they are routinely denied access to procedures and drugs that their doctors recommend.
This is so blatantly obvious that I find myself surprised to have to support it. It would be like you questioning that the sky is blue. I could do some research and post about the physics of light hitting the atmosphere, but really, if you can't see it's blue now, what's the point?
